St. Petersburg Catholic requires the following for 9th grade applicants to be considered for admission:

• Recommendation from current English, Math and Science teachers
• Recommendation from current Principal
• Official Transcripts, Attendance Record and Disciplinary Records 
• High School Placement Test (HSPT)

The following information will be needed to complete the 9th grade application:

• Name and email address for current English, Math and Science teachers
• Name and email address for current Principal
• Baptism, Communion and Confirmation dates and location (if applicable)
• IEP, 504 or Student Support Plan (SSP) documentation (if applicable)

All admitted students wishing to enroll must provide a valid Florida Department of Health Certificate of Immunization (Form DH 680) as per Florida Statute 1003.22. Religious or conscience objections to immunizations are not recognized, though properly documented medical exemptions will be considered on a case-by-case basis. For more information on required school immunizations, visit floridahealth.gov.

• Transfer Affidavit form

• Name and email address of a teacher school counselor of your choice
• Name and email address for a current administrator
