Parking Permit Request Form

Use of motor vehicles on school grounds is a privilege granted to students.  Any student who drives to school must register his/her vehicle with the Dean of Students and must have a valid parking permit.  In order to request a parking permit, the following must be completed. 

Items to be submitted to Dean of Students:

_____Completed Parking Permit Request Form (form located below or in the front office)

_____Proof of insurance

_____Copy of student’s driver's license

_____Paid $40 Registration fee through --> FACTS Tuition Portal

* Parking permit requests will not be considered valid until all items are submitted

** Parking tags must be displayed by Friday, August 25th, 2023.
